Thesis in brief

The only major private hospital system in the Bahamas, defensive and with real pricing power, and the clearest case of the two-lens gap in my coverage. To a global investor charging the full Bahamas country risk premium, a discounted cash flow puts fair value near $2.84 against a $4.87 price, roughly 42% below the tape. To a domestic investor who charges no premium, fair value is about $5.91, roughly 21% above it. The price sits between the two, so the market is charging some Bahamian risk, just less than a global allocator would.

The business

Doctors Hospital Health System (BISX: DHS) is the leading private hospital operator in the Bahamas. Demand is defensive and non-cyclical, and as the premium private option it holds real pricing power. It runs in three segments, the core Doctors Hospital (~92% of revenue), a shrinking COVID-era outpatient arm, and a growing kidney-care and dialysis business acquired in 2024. Where the value sits

  1. Two lenses, and the price sits between them. To a global investor charging the full 5.13% premium, fair value is about $2.84, roughly 42% below the $4.87 price. To a domestic investor charging none, it is about $5.91, roughly 21% above. The market is pricing a partial country risk premium, less than a foreigner would demand but more than a local strictly needs.
  2. The valuation is a country-risk debate, not a hospital debate. Change the discount rate and the answer changes, and almost nothing else moves it as much. To justify the $4.87 price you have to assume a country risk premium well below the full 5.13%, under a third of it, as if the US-dollar peg removes most Bahamian risk.
  3. The turnaround is real, and it supports the domestic case. FY25 was a small loss on pre-opening and integration costs, and H1 FY26 already swung to a $3.5M profit, so I treat FY25 as a trough. That recovery is a large part of why a domestic investor can see upside here even as a global one sees the stock as rich.
